Community Corner: Checklist for Submitting a New Group
The community team is very pleased with our recent new group growth in the under 50 market, and we have you to thank for the new business.
At ODS, we strive to complete enrollment and send out member ID cards in a timely manner when a new group is submitted. We need your help to expedite this process. When submitting a new group application, please take time to review the New Group Checklist found on the front page of the Community Master Group Application . Using the checklist will help insure that ODS has the necessary information to get your client enrolled quickly.
Following is a list of items to submit for all new 2-50 groups:
- A completed application, signed by both the agent and the group administrator
- Enrollment forms/waiver forms for all eligible employees (please make sure to include hire dates on all enrollment forms)
- The first month's premium check
- A copy of the groups last month's billing from prior carrier (used for waiving pre-x)
- A deductible credit report, if applicable
For SEHI groups (2-25), please also include the following:
- An up-to-date copy of the Speed E Rate quote (please make sure the actual enrollment matches what is on the quote so rates are correct)
- Groups with four or fewer employees must provide additional documentation (please see checklist on first page of application for more details)
